A completed Mail-In Debit Card Rebate Form with all the required fields completely filled out.
Photocopy of Sales Receipt and/or Order Confirmation Sheet for the qualifying device purchased. Please verify the purchase date is clearly visible on the photocopy.
Provide the entire IMEI / Proof-of-Purchase bar code label from the device box. On dual Rebates, the Second Rebate Form can be completed with a "photocopy" of the required
Proof-of-Purchase bar code label information. You must cut the entire UPC label from the actual device box for single rebate offers.
